To reach your personal account settings, click your user name in the upper right corner and select Account.
The My profile tab displays by default. Select the My personal menus tab to view the My personal menus page.
This section displays customized survey menus available to you or ones you created through the Survey menu functionality.
Go to Configuration → Survey menus (in the Settings column of the Configuration menu), where you can create new survey menus or edit existing ones.
